Application Guidelines
- Applications must be submitted 45 days prior to desired rotation/placement date
- Incomplete applications will not be processed
- Application does not guarantee placement
- Housing is only available for medical students, unless specified
Required Documentation to Upload
- Current Curriculum Vitae (CV)
- Copy of School ID
- Copy of State ID or Passport
- Copy of Malpractice Insurance from Medical School
- Immunization Records (MMR, Covid, Hep B, Varicella, Tetanus & TB - TB must be within previous 12 months)
- Background and Drug Screening Clearance Documentation
- Proof of Personal Health Insurance
- Copy of HIPAA Training
- Letter of Good Standing from Program Director
- Copy of Certificate or Proof of Training in Surgical Sterile Technique (if rotation includes Operating Room)
- Copy of BLS/ACLS Certification or Proof of Training (if completed)
- Employee Health Packet (please download and complete)
- Employee Background Packet (please download and complete)

Do I need to be Native American to rotate or job shadow at CNDH?
Native American heritage is not a requirement. While preference is given to Chickasaw citizens, our primary goal is to offer exceptional learning experiences to dedicated health care students.

Are housing and meals provided?
Our housing and meal support varies by program. Medical students may be eligible for housing accommodations and meal support through their university stipend. For other student categories, we recommend reviewing the details of your chosen rotation in the categories listed earlier on this page or contacting our program coordinator for the most up-to-date information.

What are the dress code requirements?
Students are required to wear plain-colored scrubs with comfortable tennis shoes. We ask that you avoid patterned scrubs or wine/maroon colors to maintain our professional dress standards.
